Key Responsibilities
- Collaborate closely with the interdisciplinary care management team to coordinate and optimize patient care.
- Actively participate in team meetings, case conferences, and care planning discussions.
- Provide comprehensive care management services, including assessments, care coordination, patient education, and follow-up.
- Communicate effectively with patients, families, providers, and internal team members.
- Document care plans and patient interactions accurately and timely.
- Support team goals by sharing insights, best practices, and contributing to continuous improvement initiatives.
- Maintain compliance with regulatory, organizational, and professional standards.

Work Schedule & Training
- Hybrid position: combination of remote work and on-site presence.
- All onboarding and training will be conducted in person to ensure a strong foundation, effective collaboration, and connection with the care management team.
- Ongoing in-person attendance may be required for meetings, training updates, or team activities.
Qualifications
- Active RN license in good standing.
- Previous experience in care management, case management, or a related clinical role preferred.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently while remaining highly engaged with a team.
- Comfort with technology and electronic health records.
