Position Summary
We are seeking a dedicated and collaborative Care Management Nurse to join our growing care management team. This role is ideal for a nurse who values teamwork, open communication, and active engagement with colleagues to deliver high-quality, patient-centered care.
Key Responsibilities
  • Collaborate closely with the interdisciplinary care management team to coordinate and optimize patient care.
  • Actively participate in team meetings, case conferences, and care planning discussions.
  • Provide comprehensive care management services, including assessments, care coordination, patient education, and follow-up.
  • Communicate effectively with patients, families, providers, and internal team members.
  • Document care plans and patient interactions accurately and timely.
  • Support team goals by sharing insights, best practices, and contributing to continuous improvement initiatives.
  • Maintain compliance with regulatory, organizational, and professional standards.

Work Schedule & Training
  • Hybrid position: combination of remote work and on-site presence.
  • All onboarding and training will be conducted in person to ensure a strong foundation, effective collaboration, and connection with the care management team.
  • Ongoing in-person attendance may be required for meetings, training updates, or team activities.
Qualifications
  • Active RN license in good standing.
  • Previous experience in care management, case management, or a related clinical role preferred.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work independently while remaining highly engaged with a team.
  • Comfort with technology and electronic health records.
